4. You will be taken to a confirmation page verifying that we have received your request. You will also receive an email confirming your request has been received.
5. You will receive another email when your request is fully processed within 45 days of submission, detailing the results of your request.

Note: Omitting information or providing inaccurate information on the opt-out form will only hinder the opt-out process. Please fill out the entire form accurately so that we can ensure we are able to identify and opt out all of your information.
